Carrie Bordinko, MD

Dr. Bordinko attended undergrad at ASU with a Bachelors in Nursing, graduating Summa Cum Laude. She managed specialty infusion/pharmaceutical and medical equipment regional offices, as well as delivered patient care as a hospice and home health nurse.

In 2002 Dr. Bordinko went on to attend Medical School at Saba University in the Dutch Antilles, and completed her clinical training at University of Missouri Kansas City, St. Luke’s Hospital in Kansas City, MO. Her residency in internal Medicine (2005-2008) was completed at St. Joseph’s Hospital & Medical Center/Barrow Neurological Institute, in Phoenix Arizona. She served as the first ever third-year Internal Medicine Chief Resident for the Internal Medicine Department. Dr. Bordinko also practiced medicine as an emergency room Physician at the Phoenix Veterans Administration hospital from 2008-2011.

Immediately after residency in 2008, Dr. Bordinko founded Consolarè Personal Physician Services with the desire to offer a very different patient experience to families – first and foremost centered on patients needs.

In her very limited free time, Dr. Bordinko enjoys boxing, water sports, weightlifting, and horseback riding.

Dr. Nadya Wayment joins Consolare.

Nadya Wayment, MD

Dr. Wayment is a board-certified family medicine and addiction medicine physician with over 10 years of experience with her patients. She practices with a passion in delivering patient advocacy, anticipatory guidance, and providing the highest quality personalized medical care to her families. Her career has been focused on developing strong relationships with her patients and their families as well as the entire healthcare team. She is looking forward to exploring a new direction in delivering superior healthcare to patients by joining Consolaré as a new physician that can offer services to all age ranges by specializing in women’s health management and pain management.

Dr. Wayment completed her family medicine medicine residency at St. Joseph’s Hospital and Medical Center in Phoenix Arizona in 2008. She also completed her Bachelor of Science in nursing in Ogden, Utah in 2002. She has a previous medical degree from her native country of Ukraine in 1999.

She holds numerous board certifications by the A.B.I.M in family medicine, and she is a member of the American Society of Addiction medicine since 2010.

Kristi Stickelman, BSN – Consolaré team associate since 2009

Kristi is a Graduate of Vanderbilt University, Nashville TN with Bachelor of Science in Nursing. She has been practicing nursing since 1984 in a variety of settings, with a particular expertise in home infusion therapy since 2002. Kristi is married and has four adult children and three grandchildren. In her free time she enjoys travel, exercise, hiking, and spending quality time with family and close friends.

Jeri Davis, RN, MBA – Consolaré team associate since 2011

Jeri joined the team as a nurse consultant; she cares for patients in their homes, and has assisted the Consolaré practice in a variety of capacities. Jeri holds a Bachelor’s in Nursing from Southern Nazarene University and a Master’s in Business from University of Phoenix. She has practiced nursing since 1988 in a variety of clinical and healthcare business roles. Her expertise lies in intensive care and pain management nursing, as well as triage. In her free time she enjoys singing and everything that involves Major League Baseball.

Kehart Flom-Racoma, MSN – Consolaré team associate since 2014

Kehart joined the team with a key interest in wound and ostomy care, as well as Diabetes management and education, alongside her Research Fellowship. She received her Bachelors and Masters of Nursing from Arizona State University, where she is currently studying for her Doctorate of Nurse Practice to become a Nurse Practitioner. In her spare time, which is little, she enjoys traveling and family time.

Lindsay Hyzer is a personal trainer for Consolare.

Lindsay Hyzer, Personal Trainer – Consolaré team associate since 2019

Lindsay comes with over 15 years of experience as a personal trainer specializing in biomechanical movements and kinesiology and as a fitness coach. She received her kinesiology degree from Northern Arizona University in Flagstaff Arizona in May 2002. She resided at Doctor’s Fitness Center for over 12 years working as a personal trainer and refining her strength and conditioning skills. This allowed her to develop a concrete understanding of the relationship of exercise and the human body for all body types and abilities.  Lindsay likes to focus on corrective exercise, balance and core conditioning as well as exercise modalities to treat osteoporosis, orthopedic injury and weight loss. Most recently, she has developed complete corporate wellness programs for clients like Barrow neurological institute and Valley Anesthesiology Consultants.

Stephanie Howland, Physical Therapist – Consolaré team associate since 2014

Stephanie graduated in 1996 from the University of Nebraska Medical Center and holds a Master’s of Physical Therapy degree. For more than 20 years she has worked as a contract therapist in orthopedic clinics throughout the valley, and more recently additionally with home health therapy. Stephanie also has a long history with the Ronald McDonald House Charities as a committee member. She is married with four children, and stays busy in their school and the community. Stephanie is also an avid runner and loves to hike.

Katherine Rutigliano, Chef – Consolaré team associate since 2017

Chef Kat received her degree in Culinary Arts from the California Culinary Academy in San Francisco in 2005. For eight years she enjoyed working at high-end catering companies in the Bay Area. After moving to Arizona in 2013 she refocused her career becoming a sought after personal chef, and now serves many families with unique dietary needs. Kat’s culinary expertise is on wholesome, organic, local, anti-inflammatory clean food. What she can’t find at the farmer’s markets, she attempts to grow in her own personal garden, and holds firm to the belief that food is fuel and our first source of medicine. Kat truly loves to cook, and also enjoys yoga, reading and quality time with her husband and two children.
